Gross report on your own customized letterhead.  Includes
       space for doctor to insert microscopic diagnosis.  Let us know if
       you need additional room for microscopic description.

Include clinical history, pre-op diagnosis and any other information
      you would like on this report.

Include number of pieces and approximate size of specimen to
      ensure an accurate gross report.

If you require evaluation of margins- specify " check margins" so
       specimen will be processed properly.  If the specimen is printed by
       a suture the specimen will be inked to preserve orientation
       information on the slide.

Diskette option:  Entire typed report on Word-Perfect format.  All
       your office does is type in the microscopic diagnosis.  Helps comply
       with CLIA regulations and saves clerical time.

Transcription option:  Diagnosis can be faxed or called in and our
       staff will properly transcribe your report and deliver a final copy.
